Position Summary: Collaborates with employer groups to manage occupational health program, worker compensation and health promotion activities. Acts as resource to patients and employer groups regarding light duty, coordination of care and return to work.

Essential Duties and Responsibilities

  1. Conducts initial needs assessment intake and scope of services evaluation for all new employer groups; summarizes findings and provides feedback on services DSH can offer.
  2. Follows up on all worker compensation medical claims, incorporating light duty options as appropriate, coordinating and tracking follow up care and appointments to expedite employee return to work.
  3. Assists employer group employees and/or dependents with the coordination of health maintenance follow up to ensure follow up appointments are made and met (i.e., physician follow up visits, lab work, immunizations, health promotion activities, etc.) based on evidence-based medicine.
  4. Collaborates regularly with patient care team for input and guidance to assure continuity of care by using analytics and reports to share information.
  5. Proactively educates employer group employees and/or dependents about self-management tasks they can undertake to gain greater control of their health status and develops programs to involve the patients in activities to improve their health. This may include internet and web based tools.
  6. Facilitates care team involvement to assess barriers when employer group employees and/or dependents have not met treatment goals and are not following treatment plan of care (i.e., not kept important appointments).
  7. Coordinates management of care opportunity reports and processes to identify areas for addressing care opportunities within the employer group. Identifies best practices among the clinical teams and communicates to all.
  8. Develops and maintains business relationships with employer groups, serving as a point of contact for resolution of issues and/or concerns facilitating employer based health promotion, disease management and coordination of care.
  9. Develops and maintains reports to ensure achievement of quality and satisfaction metrics to ensure program goals are met.
  10. Models the Divine Service Standards in all interactions and activities.

Educational Qualifications:

  • Associates or Bachelor's degree from an accredited Nursing program.
  • Current Wisconsin license as a Registered Nurse.
  • Current COHN or COHN-S certification from the American Board for Occupational Health Nurses.
  • Current American Heart Association BLS or American Red Cross CPR/AED for Professional Rescuers and Health Care Providers Certification. Non-certified employees must attain certification within 60 days of employment in position.
  • Valid driver's license and reliable method of transportation is required.
